Here’s how they break down:

Yearly Dental Membership Plan – Adult / Child

This annual plan is available for individuals and families. The plan will cover the following general and preventive services, including:

• 2 Periodic Exams
• 1 Emergency Exam
• 2 Dental Cleanings
• 2 Fluoride Treatments for patients aged 15 and under
• 2 Oral Cancer Screenings
• 1 Periodontal Screening
• Checkup X-rays as needed FMX or 4BW 2PA’s
• and 15% OFF Additional Treatment

Adults: $349 per year per person
Child (Ages 15 and under): $249 per year per person

Yearly Perio Dental Membership Plan

We also offer a second comprehensive annual plan that includes more services for periodontal (gum) related issues, such as patients needing treatment for gingivitis or advanced periodontal disease. This plan includes:

• 2 Periodic Exams
• 1 Emergency Exam
• 3 Periodontal Maintenance Visits
• 3 Oral Cancer Screenings
• 1 Periodontal Screening
• Checkup X-rays as needed FMX or 4BW 2PA’s
• and 15% OFF Additional Treatment

Adult: $449 per year per person
